Introduction and Disclaimer
The Sample Forms and Procedures included on this website are for reference and use by communities interested in starting their own Visitation Exchange Center. This is not an exhaustive listing of forms/procedures, and is not a "best practices" document. The Committee felt strongly that they wanted to share what they had learned, both from other sites across the country, and from their own experiences.

The forms are a "snapshot" in time - September 2008. Please use the Site Contact List for Sample Forms and Procedures to reach the contact person of the site that developed each form, if you have questions or want to check on updated or revised forms.
